First Aid

Who will come to save you or your family in emergency?

A. If I told you that hospital trip need about 1 hour from your place.
B. If you were a first aider, will that improve your condition?

II. Do you know how to become a professional first aider?

III. If you passed by an accident scene, would you go to help the victims.

IV. What do you feel when the patient that you helped before express their appreciation to yourself?

V. Why Malaysia does not have enough medical practitioner and first aider?

VI. Did you know that the advantages of saving life.

VII. Finally, do you know what is different between first aid and emergency?

VIII. Now, I would like to share my experience with you and told you why we must practice first aid.

I. First purpose of First Aid is to preserve life.


A. Human has been creating by God to live in this earth without any accident.

1. Human already exists long time ago but we still do not know when exactly humans have been here.

2. In Malaysia, no people are allowing to suicide himself. If anybody does so, he / she will be charge against the country law.

3. That means life is important, we have no rights to take others people life. Instead, we have to save human life as many as we can.


B. The priority and the principle of preserve life.

1. Airway (A) - We try to opened the airway of the patient and make sure no obstruction objects inside his/her mouth. Besides, we try to Head Tilt Chin lift to allow more oxygen go through his/her mouth.

2. Breathing (B) – We try to check his/her breathing system whether is exist or not. If the patient does not have, any signs to show that he/she were breathing, so we have to perform mouth-to-mouth ventilation to supply oxygen by the correct method and procedure.

3. Circulation (C) – We will check his/her circulation through the pulse that is located in different place and different name.

a. Carotid -Neck
b. Brachial -Arm
c. Radial -Wrist
d. Femoral -Hip
e. Temporal -Ear

Normally, an adult person will show the pulse rate between 60 and 80. Children and infant have different rate. Besides, we can survey and look at their skin color to reconfirm his/her circulation system. We have to make sure that the patient has sufficient circulation and we must restore the circulation when needed.

(Transition: When you arrive in an emergency scene, you must access the situation, make the area safe, give emergency aid, and get help from others no matter they are professional or not.)


II. Second purpose of First Aid is to prevent condition deteriorating.


A. Triage


1. Some patient will get easily dead if the first aider do not know the priority of saving life or treatment that should be give to the patient that are seriously injured. Anyway, DRABC should be performing on the beginning like what I said in stage 1.

2. First aider will analyze the degree of urgency and the casualty groups. As a professional first aider, he/she knew that casualty management had four kind of priority.

a. Priority 1 (Red) -requiring urgent transport and treatment.
b. Priority 2 (Yellow) -requiring surgery within a few hours.
c. Priority 3 (Green) -requiring surgery when possible.
d. Priority 4 (White) -Death and no requiring at all.

3. In medical field, they have one examination that we called as “Top to toe examination”.

a. Primary Survey
First Aider will check his/her eyes, nose, mouth, ears, face, skull, neck, shoulder, collarbones, spine, arms, pelvis, and legs.

b. Secondary survey
First Aider or paramedics will make decision whether the patient need any oxygen therapy, fluid resuscitation, analgesia, urine output or other internal injuries.

B. First aiders have easy steps to prevent condition deteriorating.

1. Dress wounds. We have to treated the wounds and escape from continuing bleeding. If we had loss about 1 liter of blood or fluids from our body, we will be weak and fainted. In additional, if human loss 2 liter of the body fluids, definitely he/she will be shock and might be dead or paralyze.

2. Immobilize fractures. If one of the victims had met a leg broken, we should not move him/her because it is not a good way to prevent condition deteriorating unless we have the medical aids likes air begs that can support the broken leg.

3. Correct and comfort positions. We have to make sure that the patient in correct positions in different case or injuries. Normally, first aider will put him/her in recovery position.


III. Third purpose of First Aid is to promote recovery.



A. This was final level for first aider to reconfirm that patient had received a complete professional treatment that can cure some injuries part before any medical practitioners or medical assistant is in-charged to the patients.

1. First aiders have to send the patient or victims to the nearest hospital or clinic to get further professional treatment.

2. First aiders also have to send the report regarding to the condition of the patients. The reports actually is a diagnose that includes the history, sign and symptoms and external clues.

B. The ways that first aiders took to promote recovery systematically.

1. Reassure and promote confidence. Sometimes the first aider will meet the raped patient. Therefore, the first aiders have to fight with her feelings by advising. Anyway, this kind of patient needs speech therapy from the psychiatric Doctor because she was so scared and shock after the bad incident. First aiders also have to tell the patient that her parents were on the way to meet her. Family was a kind of support for some patients.

2. Relieve pain and discomfort. First aiders have to find a calm place to sit him/her down. Besides, they also have to relieve the pain of patient such as raped patient. We have to bend her knees and let her lying down on the surface to reduce the degree of painful.

3. Gentle handling and protect from cold. First aiders have to make sure that the patient was not cold. If he/she was cold, supply a blanket for her. First aiders are not allowed to supply any foods to the patient because its will block the next procedure and treatment process. That means sometimes first aiders also should be strict to the patient.

(Transition: As we know, if someone met an accident, he/she would be shock and blur-blur. That is why we have to talk to the patient to find back his/her confidence. Finally, you know that the responsibilities of a first aider is not only curing the injuries but also includes mental cures.)


Please remember that first aid purposes are to preserve life, to prevent condition deteriorating and to promote recovery.

In determining circulation, you suggested the checking of pulse, from pressure points that you've described. Since 2000, however, AHA, ILCOR and other leading de-facto authorities have suggested otherwise. Hence, what' the best way to deal with determining the presence of circulation?

Besides, I thought the need for oxygen therapy and IV mainly deals with complications and disorders associated with breathing and circulation, and in most cases, we're dealing with life threatening situations. Isn't it part of primary survey rather than secondary survey?
